Department of Motor Vehicles (DMV) Information

After cataract surgery, you may no longer need to wear eyeglasses to drive. However, your driver’s license may indicate that corrective lenses are required. In order to have your license corrected, you will need to complete the necessary form and bring it to your DMV facility.

It is important to note that every State has a different form. If you are licensed in New Jersey or Pennsylvania, please print by selecting NJ or PA below and complete the appropriate form.

Please bring the form to your final post cataract surgery appointment so your eye surgeon can sign the required form. If your driver’s license was issued outside of New Jersey or Pennsylvania, please contact your DMV office. Enjoy your great vision!

  • Request that your medical records be released to someone else.

Note: To protect the confidentiality of our patients, we can only fax medical records in extreme emergencies. Please plan ahead to leave enough time for records to be mailed.  Please note that it may take up to thirty (30) days for records to be sent.
